CHQR awards recognize Health Care Program awardees that made notable quality improvement achievements from the most recent reporting period. In the last reporting period, CHWP served more than 8,200 patients for a variety of services, including mental health services and substance use disorders. CHWP received the Silver HRSA Health Center Quality Leader Award for achieving the best overall clinical quality performance measures among all health centers nationwide. CHWP earned the Silver tier, ranking in the top 11-20 percent of health centers nationally.
Supporting CHWP’s mission of increasing access to health care, CHWP was recognized with the Advancing Health Information Technology (HIT) for Quality. CHWP had to meet all HIT services criteria, which included advancements in telehealth, patient engagement, interoperability and collection of social determinants of health to increase access to care and advance quality of care. CHWP also earned the Patient-Centered Medical Home (PCMH) Recognition. This acknowledgment is given to health centers that achieve PCMH recognition in one or more delivery sites. CHWP currently has three locations, with a fourth opening in Q1 2022 in Urbana, Ohio. Earlier this year, CHWP launched a school-based health center and a mobile unit.
