Service and Support Administrator

Job Description: Join our team as a Service and Support Administrator where you'll be responsible for assessing individual needs, coordinating Individual Service Plans (ISP), advocating for enrollees, facilitating team processes, and managing service delivery to ensure that individuals receive quality care and support.

Key Responsibilities:

• Develop and coordinate Individual Service Plans (ISPs) and facilitate person-centered planning.
• Advocate for enrollees and their families, ensuring they know their rights and have access to self-advocacy resources.
• Facilitate team meetings, ensure plan updates, and focus on the enrollees’ personal goals and outcomes.
• Maintain communication with enrollees, families, providers, and community resources.
• Monitor service delivery, identify gaps, and make necessary adjustments.
• Ensure Medicaid billing and maintain documentation for compliance.
• Perform other related duties such as incident reporting, quality review, and professional development.
