​General Job Description​
The role of the PATH Navigator is to create, document, and revise personalized service plans and records, to ensure client daily needs and desires are met.

Employment Specifications
  • Tier 2: $14-$18 per hour
  • 40 Hours per week; Salary

Major Duties and Responsibilities
  • Monitor, support client care as needed. UIs, documentation etc.
  • ​Connect with individuals, SSA or family to ensure client care.
  • Review, gather information, investigate, and inform county and families of incident reports.
  • Ensure client care in plan of action for crisis.
  • Communicate with staff, parents, and SSAs to discern and distribute relevant information on each client.
  • Run the client profile venue to gather personalized info on what is important to and for the individual. Encourage clients to present their Profile book to their peers prior to their ISP meeting.  If necessary, meet 1:1 with the individual to gain greater understanding of needs, desires, and barriers.
  • Make personal schedules with upcoming meetings, site visits, due dates for documentation, ISP meetings, etc. Use the 90, 60, 30 calendar to set goals to complete all objectives in the path, annual and enrollment process.
  • Reconcile Personal Funds Ledgers.
  • Gather information from discovery, daily documentation, and PATH assessments to create personalized ISP services. Talk with clients about their successes and barriers.
  • Attend Annual ISP meetings and assist client in self-advocacy, goal planning and team relationships throughout the meeting
  • Update Brittco with Client information. Using the ISP Review Tool, review ISPs and notify SSA of edits needed. Hold SSA accountable to send the ISP in a timely manner.
  • Train other DF staff on current individuals' ISPs, Medications, personal funds, or other relevant ISTs.
  • Gather Path Assessments, score, and create Path report. Fill out Path Aval prior to path meeting and send email to team of client concerns. Finish Eval and complete evals. Turn into Client profile designer for filing.
  • Review Client notes, goals and examine for progress.
  • Work with Client Profile designer to develop progress visuals for clients.
  • Complete required HR Trainings
  • Review Quarterly ADS schedule and offer suggestions as needed. Notify clients of changes.
  • Participate in required QAs, and POCs as needed.
  • Report Individual success in the OTS system for DODD.
  • Review Billing errors and audit for utilization errors. Ask for revisions as necessary.
  • Work with Residential team to sign off on site reviews and hold accountable for site documentation.
  • Perform Direct Care as needed.
  • All other duties as presented

Additional Responsibilities
  • Must attend Mandatory Quarterly Training Classes- provided by Downsize Farm
  • Must maintain updated First Aid/CPR training and provide HR with a copy.
  • Must maintain Med Admin Certification, if applicable and provide HR a copy
  • Provide the HR Manager with a copy of the updated Drivers ID, as applicable.
  • Provide the HR Manager with a copy of the updated Private Auto Insurance, as applicable.

Qualifications​​
  • Associate degree or equivalent experience.
  • One year or more experience working with individuals with disabilities.
  • Meets eligibility and certification standards of DODD for Adult Services Employment

Daily

  • Staff correction/conduct
  • Interview new staff
  • Find coverage for call-offs
  • Work with Staff in crisis and MUI/UI Investigations
  • Retrain staff as needed and ensure POC's are being followed
