Memorial Health is actively seeking a Licensed Practical Nurse (LPN) to join its team at Urbana Family Medicine as a Care Coordinator, offering a $5,000 sign-on bonus for qualified candidates.

Position Overview:
The LPN Care Coordinator will support multiple providers and work directly with patients to help them set healthcare goals, complete advance care planning, and track provider quality goals. Candidates should be comfortable using Microsoft Office and be proficient with data tracking and reporting tools.

Qualifications:

• Active Ohio LPN license
• Nursing degree from an accredited school
• Office experience preferred
• BLS/CPR certification preferred
• Proficiency with technology required for data tracking and reporting
