Early Intervention Service Coordinator

Champaign County Board of Developmental Disabilities is seeking a full-time Early Intervention Service Coordinator.

Qualifications:

• Minimum of Associate degree in early childhood, special education, or related field required
• Early Intervention Service Coordination certification must be obtained through Ohio Department of Developmental Disabilities
• Knowledge of early childhood and special needs
• Ability to establish and maintain rapport with children, parents and professionals, define problems, and collect data. Prepare and maintain meaningful, accurate and concise records and reports
• Appropriately handle routine and sensitive inquiries and contacts from professionals, parents, and guardians

Duties:
The Early Intervention Service Coordinator (EISC) functions as a transdisciplinary team member in the early intervention program offering specialized services to families of children who have a developmental delay or a qualifying medical diagnosis.

• Interacts with the family and child in their natural environments
• Preliminary point of contact with families for all Part C Early Intervention Services and will provide referral information gathered to the team members during team meetings
• Conduct follow up visits with the family and provide a Family Directed Assessment to the team. All contact with the family will be documented and completed in a timely manner as to accommodate the need for shared documentation with other team members
• Empower families to identify and access resources on behalf of the child and their family, advocate for the child and family’s rights and needs, and assist parents in gaining access to services including making referrals to other social service agencies
• Complete required prior written notices and screenings per Ohio Early Intervention rules with the parents consent
• Document and maintain all required data needed and enter in the statewide data system within a timely manner as required by Ohio Early Intervention rule
• Complete all documents required by Ohio Early Intervention rule. Ensure these documents are accurate, thorough, and signed by parents. Send documents, as required, to service providers and other agencies
• Regularly communicate with the Early Intervention Director and team members regarding services and duties
• Maintain and foster communication with the family and other members of the interdisciplinary team to assure needs of the families/children are being met
• Under the supervision of the Early Intervention Director, the primary function of the EI Service Coordinator is to initiate the development of the IFSP with the family and team and to coordinate those services on the IFSP. The EISC will assist the IFSP team with developing outcomes that are functional and reflect parent concerns and priorities
• Coordinate and monitor services and adhere to the required timelines. Ensure all providers of service are engaging and completing services as listed on the IFSP
• Attend meetings, trainings, staffing, etc. They may serve on agency committees and conduct seminars/workshops for agency personnel
• Perform other related duties as assigned

This position is dependent on grant funding. If grant funding is reduced or eliminated, the EISC position is subject to change or abolishment.
