CRSI is currently seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join its team as a Full-Time Night Shift Field Associate. This opportunity offers the chance to provide essential nursing services and ensure the well-being of individuals in need.

About the RoleThe primary responsibility of the Field Associate is to deliver quality nursing care to consumers based on their individual medical needs and personal plans. The role ensures compliance with the standards and regulations established by DODD Medicaid (Title XIX), state DODD Licensure, County Quality Assurance, and other governing bodies.

Key Responsibilities

• Follow agency policies and maintain accurate records.
• Ensure consumer safety, health, and well-being.
• Administer and monitor medications and treatments as prescribed by physicians.
• Conduct routine health assessments and participate in annual meetings.
• Communicate with physicians, pharmacies, and healthcare professionals to coordinate care.
• Participate in admission and discharge planning.
• Maintain a clean and safe environment while being sensitive to consumer and family needs.
• Assist in staff training and ongoing monitoring of delegated nursing practices.
• Participate in infection control programs and ensure proper medication storage and supply.
• Provide on-call support as needed.

Qualifications

• Active LPN or RN license in the State of Ohio.
• Valid driver’s license and vehicle insurance.
• Acceptable background screenings and police record.
• At least one year of experience in adult care preferred.
• Professional Liability Insurance is recommended.
