Job Summary:
The Champaign County Family and Children First Council is seeking a dedicated individual to serve as the Multisystem Youth Program Coordinator. This position involves acting as the Child & Family Team Facilitator for multisystem youth and their families. The coordinator will be responsible for developing a Plan of Care, conducting assessments, building family teams with key stakeholders, and ensuring accurate documentation and reporting for the service coordination program.

Key Responsibilities:

• Facilitate team meetings using the Wraparound approach.
• Conduct CANS assessments for youth and families.
• Develop and implement comprehensive Plans of Care.
• Maintain accurate documentation for all enrolled families.
• Provide monthly reports to the State and local Family and Children First Council (FCFC).
• Support the Director in managing program operations and financial aspects of the Council's various programs.

Qualifications:

• Minimum of an Associate degree in social work or a related field, or equivalent college credit hours.
• Bachelor's degree preferred.
• Two years’ experience facilitating team meetings with the Wraparound approach.
• Experience working with families, children, and community organizations is preferred.
  
  

Compensation:

• $18.00 - $22.00 per hour, based on experience.

Overview: As a PATH Navigator, you will create, document, and manage personalized service plans to meet clients' daily needs and preferences. You will work closely with clients, their families, and other service providers to ensure comprehensive care and effective support.
Key Responsibilities:

• Monitor and support client care, including documentation and incident reporting.
• Communicate with clients, SSA, and families to ensure proper care and address concerns.
• Develop and update personalized service plans (ISPs) and gather necessary information.
• Attend and assist with Annual ISP meetings, focusing on client self-advocacy and goal planning.
• Maintain accurate client profiles and manage related documentation.
• Train staff on ISPs, medications, and other relevant topics.
• Perform direct care duties as needed.

Qualifications:

• Associate degree or equivalent experience.
• Minimum of one year of experience working with individuals with disabilities.
• Must meet DODD certification standards for Adult Services Employment.
• Valid Driver's License and up-to-date First Aid/CPR and Med Admin Certification (if applicable).

Additional Requirements:

• Attend mandatory quarterly training classes provided by Downsize Farm.
• Maintain updated certifications and insurance documentation.
